Appointment reminders:

• Please arrive at least 15 minutes early to your scheduled appointment. If you have to fill out paperwork in office, please arrive 30 minutes early.
• Photo ID is required for each Patient at every visit.  We accept:  Drivers license, state issued identification only cards, US military identification cards, passports, consulate identification cards, and local college identification cards. 
• Please remember to bring your insurance card with you to your first appointment or whenever you are issued a new one or change insurance companies.
• For private pay patients, please bring the total amount due for your appointment, with you to your appointment. Please call our office to arrange a payment plan before your appointment if needed.
• Turn off or silence your cell phone upon arriving.
• If you have a cough or fever, please wear a mask provided as you enter the facility.
