You understand that MyHealth contains protected health information (PHI) and agree to keep your login credentials confidential.
You will not share your username, password, or any accessed information with unauthorized individuals. -
Compliance With Law:
You agree to use this system in accordance with all applicable federal and state privacy laws, including HIPAA. Unauthorized access, sharing, or misuse of health information may result in civil or criminal penalties. -
Monitoring & Revocation:
You understand that system activity may be monitored, and access may be suspended or revoked at any time for security or compliance reasons. -
Responsibility for Use:
You accept full responsibility for all actions taken under your account while logged in.
